This form is intended for use in the Pastoral Staff annual review process at the church. Please only fill out the form if you have been assigned the task by the chair of the personnel committee. If you have been assigned to fill out a review for a non Pastor, please use the non-pastoral staff review form. The Pastoral staff forms that you fill out online, when you press "submit", will be emailed directly the chair of the personnel committee who will meet with individual staff members and review the information that is collected.
